LFW Podiatry / Chiropody & Wellness Clinic in Burley in Wharfedale, West Yorkshire began in 2011 by Lynda Forbes-Wake and has grown from a home visiting service to a purpose designed clinic at 113 Main Street, Burley in Wharfedale. Lynda has, in the last year been joined by Associate Podiatrist Helena Meally, who is qualified to the same high standards. We both graduated from The University of Huddersfield with BSc (Hons) degrees in Podiatry in 2011. We are registered with the Health Care Professions Council (HCPC) as well as being full members of the Society of Chiropodists and Podiatrists (MChS).
We hold a POM's licence (Prescription Only Medicines) and are licensed to administer local anaesthetics. Being members of the HCPC means we follow a strict code of conduct and update our skills via CPD (continuous professional development) on a regular basis. Other advantages are, if you are registered with insurers such as Sovereign Health and Simply Health, you can often claim back a percentage of your treatment.

Data Collection and Retention
We collect certain data from you to meet mandatory requirements regarding medical notes. We have a legal obligation to keep medical records for a period of time after any treatment carried out. The length of time your data is kept can vary, depending upon your age and ability to consent, but there is a minimum period of seven years. After this period your details will be destroyed.
In addition, we collect data to assist in the administration of the business to ensure we provide our patients with an efficient service.
Reason for Consent
We are obligated by our governing body to ensure we received consent from patients before any assessment or treatment is carried out by a HCPC Registered Podiatrist.
Due to the equipment and instruments used during your treatment it is necessary for patients to be fully informed and have full understanding of any procedures carried out.
We request your consent should we recommend photographs be taken for the purpose of recording the progression of your treatment. Any photographs taken will not be used for advertising or marketing purposes.
On occasions, we may need to share information with your GP or other healthcare providers, in order for us to refer you for continuation to the treatment being carried out. This can only be done with your consent. No data is transferred to a third party without the patients consent.
We request your consent to ensure you are happy to be contacted by text / email with reminder information in relation to appointments/treatment.
In addition, your consent is required for payment of the treatment. You are signing to confirm that you are able to pay for your treatment in full at the time of the appointment and are happy for any missed/cancelled appointments to be charged in full if less than 24 hours’ notice is received. We fully understand that on occasions, illness and other urgent situations may arise where we will discuss a waiver of the cost of the missed appointment.

All data is stored securely. Paper records are filed and stored within a lockable cabinet. Electronic data is stored locally and on an external hard drive.
Our our online booking system data is securely stored in a backed up, encrypted cloud database. Shedul’s system uses SSL security and the databases are encrypted and the hardware has top of the range firewalls to ensure it is well secure and stable.
Access, Modification and Deletion of Data Records
Data records (paper/electronic) are kept up to date following each treatment.
Patient files are deleted after seven years if they no longer attend regular appointments. Patient records are deleted from the data processor and external hard drive and paper records are shredded.
